2011-12 Speedway Australia Application Forms are now available to download.
2011-12 Speedway Australia Application Forms are now available to download.
The licence forms are in PDF format and include the compulsory Health Statement, as well as a Medical form that can be filled in by your medical practitioner.
All 2011-12 Licence Applications must be completed using this form.
The 2011-12 Licence Forms will be distributed to Speedway Australia State Branches, associations and clubs shortly in limited supply.
Printed forms will continue to be phased out over the next several seasons in favour of the Speedway Australia Online Licensing System (to be launched during 2011).
